Calhoun County Transition Council
Community Transition Councils are action-oriented groups with an identity established by their targeted activities. There are numerous activities that can become the focus for community transition council committees. The type and direction of projects and activities should be determined within the local community in order to obtain outcomes that reflect the community's needs.

Vision:

The vision of the Calhoun County Transition Council is to collaborate within our community and support opportunities to build a path toward a successful future for each individual.

Goal Statements:

  • Encourage individualized planning as an essential component of the transition process.
  • Identify issues, problems, and systemic barriers to effective good transition services, then collaboratively plan ways to overcome them.
  • Expand and develop programs and services that emphasize the inclusion of people with disabilities in all aspects of their community.
  • Share information, needs, resources, philosophies and ideas.
  • Identify existing community and school resources and services that are available for individuals involved in transition efforts.
  • Establish a system to facilitate regular and routing communication among consumers, students, parents, educators, consumer advocacy groups.
  • Develop or gather, then disseminate information on transition.
  • Designate liaisons for transition planning among schools and agencies.
